Registration Information

Registration fee is $150.00 for Physicians, and covers all course materials and group meals and breaks.  All Industry Participants and Sponsor Representatives should contact Jennifer Heald for registration information.

Registration is limited and is on a first come, first serve basis.  Please register as soon as possible, but no later than October 15, 2010.

The first function begins on Sunday, October 31, at 2:00 pm. The last function ends at 12:30 pm on Tuesday, November 2.  A complete tentative agenda may be found on the Agenda tab of this registration site.

Please understand that all functions are of a business nature and are not suitable for spouses and/or children.

Registration Cancellation Policy:  Cancellations received in writing by September 27, 2010 will receive a full refund minus an administrative fee of $25.  Refunds will not be available for cancellations received after September 27, 2010.  Hotel cancellation policies are separate from the registration cancellation policy.
